Legal Question in Civil Rights Law in Utah
I am looking to have a friend be a sperm donor. He wants a waiver and agreement giving him no rights or anything, which I am completely fine with. How or what forms do I need to present or give him to make it so he will not have any custody or have to pay? I live in Utah. Please help!

Sperm donor should contact an attorney to make sure he understands his legal rights and potential consequences. There are no "forms" to sign, a specific contract will need to be created.
