Legal Question in Consumer Law in Utah

I work for a company that deals with long term contracts (2+ years) and charges a monthly rate between $25 and $75. The majority of our customers pay automatically through a bank account or credit card. Occasionally, small groups of customers are not billed for months at a time due to an accounting error. The error is eventually discovered (usually no more than 4 months later) and customers are back-billed immediately with no notice. Obviously, this is poor customer service, but I'm concerned it could violate consumer protection laws or statutes. While the company is based in Utah, there are customers nationwide and in Canada. Does it depend ono local laws or is the contract stating that monthly payments are due sufficient?
