collections advice

Is it legal for a doctor's office, that I have never been to before, to charge for a missed appointment and send it to collections for payment?

You can contest the charge; demand in writing from the collection agency proof of your agreement to incur and pay the debt. Also tell the collection company to only communicate with you in writing - no phone calls. Makes life easier. Also file a complaint with the doctor.
