Legal Question in Family Law in Utah
I'm 20 years old and a single mother of a 4 month old little girl. I was never married to her father and paternity has not been established. I tried having the father involved with her and he says he does, but hasn't shown it at all. He hasn't come to see her, hasn't asked about her, nothing. Hasn't provided for her at all since she was 2 weeks old. He is threatening to take custody from me because I feel if he's not going to try and be there for her as her father than he shouldn't be in her life at all. He currently has a girlfriend who just barely found she is pregnant with his child, and he has no stable place to live, no job, recently got arrested for a D.U.I. and has many other charges on his record that scare me for him to have custody of my child. Can I go about getting full and sole custody and giving him visitation rights at my discretion and have the visits be supervised because I do not trust him alone with my daughter since he threatened to take her out of state without my knowledge.

First, he can't "take" custody of your daughter from you. You should file a paternity action against him in court to get paternity, custody, visitation, and child support established. I offer a free initial consultation if you would like to meet to discuss your case further.
