Legal Question in Family Law in Utah

I have a 3 yr old son, who I gave my last name when he was born. I am wanting to add his biological fathers name to the birth certificate as either a hyphen or as a middle name. Will this cause any legal issues for him in the future if he chooses to go by just one of the names? Does he always have to use both and do we have to change his social security and passport to match even if it is just a middle name? Any advice on what to do?

You can apply for a name change for the child. The new name you choose will be his new legal name.
