Legal Question in Family Law in Utah

I am 60 yrs old and have a 37yr old adopted son who is mentally disabled. If something happens to me I would like my other son who will be 21 in June to be his legal guardian. I am in the state of Utah. What do I need to do to ensure this will happen?
