How do I change my name if it was not specified in the decree?

My husband filed the divorce papers, he thought I was keeping his last name. I thought I just took the papers stating I was divorced to change my name, I had no idea it needed to be included in the decree. He told me this after it was finalized and I stated I needed a copy to change my name. How do I go about getting my name changed or submitting a change?

File a petition to modify the decree, to restore your maiden name.
