name change to maiden name

Would like to change my name back to maiden I am still married. Either hyphen it or take maiden name back. Would prefer maiden name.

Re: name change to maiden name

You must file an application for a name change with the State district court for the county where you live. You will need to attest to the court that you do not have any improper purpose for the name change, such as to defraud creditors, avoid responsibity for your acts, etc. You can do this yourself. Go to a court and ask the clerk to review a copy of a name change file. Duplicate the forms using your information. Good luck
