Changing my sons name

I have a seven year old son who currently lives with me. We have not seen or heard from his father in over 5 years and vary rarely before that. He does not pay (nor has he ever had to) any child support of any kind which is not wanted anyway.My son currently has his last name, but being as thought he is never around and does not plan to be, I would like my son to have my last name. This would make it easier for school registration, medical insurance, beneficiary papers, plus make my son feel better as he goes by this last name when he can anyway. Can I legally get his last name changed? What steps would I have to take? I am sure that if contacted about this matter the father would not have any problem with this (If I knew where to contact him).

Under Utah law your ex is considered to have abandoned your son. You can apply to have the ex's parental rights totally removed, then apply for a name change. You should have no problem with this.
