How do I go about filing a motion for recusal? Is there a form, of can I go to the court house to file it? I live in SLC and my hearing is in Heber. Do I need to go to Heber to file it?

If your case is in Heber, you will have to file your motion there. The courts generally don't have forms for that sort of thing. Normally, most people retain an attorney to represent them in that sort of matter.
