New Court Order needed for custody and child support

Ex sole custody/physical residence. Daughter has lived with me for over the past year and does not want to live with her dad. He agrees and will not fight custody/ch support change. What is first step I need to do? Ex told me to do the paperwork and he will not fight it. Been divorced 18 months. Simple chnages needed - sole to joint and new ch support calculation. Thank you
