Custody decree not good for kids

the decree states that my ex and I will share the kids every 2 school years. He moved to a different city but the decree also says he has to make efforts to live in the same city prior to taking them. (we are about 35 miles away) He hasn't and when it came time for him to have them for his 2 yrs we talked and both decided that it was best to leave the kids with me. They are on honor roll and doing well. They live in the house they grew up in with established friends, etc. Ex doesn't take them when he is supposed to, threatens to tell them of his alternative lifestyle (he wants a sex change) and has now threatened to take them out of the middle of the school year for his 2 years. He lives in a 2 bdrm high rise apt (we have 3 kids) in the middle of downtown SLC. No yard and he works nights. He sleeps during the day while they are there. How do I get this under control? Emergency hearing? Should I involve DCFS? I know I can't stop him from taking them due to the decree but I need to know how to immediately address the issues until I can try to get the decree revised. He is not emotionally healthy and it would be detrimental to the kids. HELP!

You must file for a change in the court orders and a temporary order of custody.
