Divorce & Quit Claim Deed

Ex was awarded home in divorce. The home quit claim deed still reflects both names. What is required to reflect the actual occupant of the home? When the settlement is paid per decree, does this automatically assist in removing the name from the property title, or would this be an additional paperwork step required?

Add'l paperwork is needed to remove name from title. A new loan will be needed to remove name from loan.
