Legal Question in Family Law in Utah

giving my mother custody of my child

my child is out of control and my mother has had her live with her in Iowa recently and I got a letter notorized for her to have custody but need to get done through the courts. I wants to draw up the paperwork myself is there a special form or process I needs to follow to get it done ASAP and cheap? I do have sole custody of my daughter. please help!!!

Locate the nearest Utah Legal Services agency, who can help you with the paperwork. You will want to give your mother temporary legal custody. There is no simple form.
