Involuntary termination of parental rights

my ex-husband is a sex offender I cant afford a lawyer a court clerk said that I can terminate his rights with one form with out a lawyer due to the fact that his victim was our infant daughter I cant find the form. In the state of utah will I be able to get rid of him with one form or do I need a lawyer? and How do I find either a cheap good lawyer or a pro bono lawyer? can he fight it does he have a legal leg to stand on?
