Legal Question in Family Law in Utah

I married my husband 2 1/2 years ago and he would now like to adopt my 13 year old son. His biological father has not made contact with my son in almost 4 years and has only exercised visitation minimally for 2 years of the 13. He has not regularly paid child support the majority of our sons life either. We are wondering how to start the process of requesting that my son biological father waive his parental rights, and how to go about having my husband adopt our son legally.
