Are there any blank forms to fill out requesting a mediation hearing or how should I ask (legal writing format)

Unfortunately, the legal/judicial process is much more complex than filling out forms. Frankly, it's no more advisable to be involved in litigation without an attorney than it is to perform surgery on yourself.
