Legal Question in Family Law in Utah

Qdro

I was divorced in 2002, in my divorce papers I was to get 50% of my husbands pension. He recently died and his pension people have gotten ahold of me to fill out a QDRO, which I am not sure how i do that... or possibly I could have done it when divorced, but cannot remember.
