Legal Question in Family Law in Utah

Both I and my x-wife have agreed to the terms of the modification on our divorce decree that was finalized 18 plus years ago, however I can not find the correct form to file and or is it in our best interest to have an attorney file the paper work for both parties as there is no disputing to the change? Any insight would be greatly appreciated.

There isn't a "form" which is used. You would normally need to have an attorney assist you in the preparartion of the necessary documents.
