How do you write a change of contract? What is needed? How is it accomplished?

One references the original contract and rewrites the terms that need to be changed, and all parties sign it. That is the simplified version, you should talk to an attorney to make sure that it is done properly for the type of contract - for example a real estate contract is different than an employment agreement.
