Sueing a Company

what forms do I need to file if the money is 10,100.00 + interest. The

company we turned in was for not providing the service that they stated in the contract Utah Department of Commerse file fines on the company and told them if they gave back the money the fines would be minimal but they missed the court trial I need to pull them into courtto get our money back
