Legal Question in Civil Litigation in Utah

tempory guardianship

I live in California I want to send my daughter to Utah to live with her uncle and finish out the school year. does Utah have a JURAT FORM- Caregiver's Authorization Affidavit like they do here or some other form or do I have to sign over tempory guardinship. My daughter has been bullied and attempted suicide spent 4 days in the hospital and the school wont do anything about the bullying so I have no choice but to move her unfortunately. You might get by with a notarized appointment, but the preferred method is by temporary guardianship through a court
