Involving wills and Probate

In the State of Utah to file for a Informal probate process you need to use a form called, APPLICATION FOR INFORMAL WILL AND FOR INFORMAL APPOINTMENT OF PERSONAL REP. the problem is, there is another form called, APPLICATION FOR INFORMAL APPOINTMENT OF PERSONAL REP. do i need to use both forms or just one? is there another form i can use to file for informal probate and then file for a informal personal rep later?

The form to file for the informal probate of the will allows you to do so in the alternative--you can petition the court to let you probate the will informally, appoint the personal representative informally, or both. I always do both--nothing in the probate can be done until the personal representative is appointed. If you have questions, please call me tomorrow.
