If I do a irrevocable Trust, Do I also have a need for a revocable A/B Trust

If I do an irrevocable Trust, for Medicaid and or legal protection, do I also have a need for a revocable A/B Trust? Because of the need of guardians, Durable Power of Attorney, Medical Directive and Medical Power of Attorney and etc.
