Insurance Acord 60 - Flood Insurance Notice/Rejection
This form has a single line for Applicant signature and date, the space for property address, etc. If policy has 2 named insureds (spouses for instance), are BOTH signatures required to indicate that they do NOT wish to have Flood coverage? We have mailed these forms to clients requesting signatures of both insureds, but still receive form back with only one signature. Is it sufficient to accept the one signature, or should we continue the practice of sending a second time to get the missing signature?

The standard Form 60, 2000/2008, only has a signature line for one insured (applicant). If you use that, why not just send two, one for each person. It is far safer to get two signatures, particularly when the rejection is for all future renewals.
