Insurance Acord Forms

If form only provides one line for signature and date, does that mean that only one of the insureds is required to sign such a form - legally??

An ACORD form is issued by a broker for an insured, and it is not part of the policy or necessarily binding on the insurer. It is for information only. Insureds do not sign it, it is sent to show a person is an insured. But if you are supposed to be an insured, the only way you can be sure is to have the policy itself, or the endorsement from the insurer, naming you.
