Do I have to have an attorney write the letter
This is a question related to your suggestion to get a lawyer to help me with a letter to my employer regarding my ailments and my request for reasonable accomodations. Must I have a lawyer or may I compose a letter myself and see what type of response I get first? If my employer does not grant my request, then I will seek an attorney at that point.

By all means, compose the letter yourself, if you believe yourself capable, and send it to your employer.
(That's what LawGuru.com is all about, i.e., providing folks with direction as to how to first address their legal issues themselves, if at all possible, before involving lawyers.)

Attach to your letter whatever appropriate documentation that you may have from your doctor in support of your claims/requests.
