I believe that what is intended by "Name" is the same as your signature in "BY" except that it is written out so that it can be read, rather than a signature.

So you would put the same name in both places, but your signature would be next to "BY" and your name spelled out would be in "Name"

If you are really paranoid about it, you could write "BY" again in front of your name written out.

However, whether you have personal responsibility is also controlled by what the contract actually SAYS.

If it clearly says that it is a contract between the XYZ company and the ABC company, then what the contract SAYS is legally binding on that question.

So also examine the opening paragraphs and any other provisions in the contract that might be relevant (such as "personal guarantee" !!!)
