Virginia code statute 20-111.1
Where can I get the form for Beneficiar designation?
I am divorsed person, not remarrige in Virginia. I have a large amount in life insurance and according to the law in virginia, my ex-wife can not be the beneficiary anymore, unless there is a separate written agreement. I am looking that particular form or agreement according to the code listed on the subject part.

You should be able to get the form for designating
such a beneficiary by sending a written request to the life insurance carrier which issued the policy.
