How do I make a sworn response to a pleading?

Appear before a notary, and, after presenting appropriate ID, execute the required oath pertaining to the subject pleading before the notary with signatures and notarial stamp then affixed to the document attesting to the oath and notarization on _________day of April, 2008, in city/county of _________, commision of notary expiring,_____________ etc.
