i have custodey of a family members child and i want to adopt where can i find the forms to put in a motion for the courts

You could call the clerk's office of your local J&DRDC which granted you custody and see whether they have forms available for adoption. However, in the event that they do not, you will likely need to retain the services of an attorney to prepare and file the adoption petition.
