Appeal Custody Order

Recently I went to court for a modification in custody and visitation. Visitation has not been decided as a GAL has been appointed to decide what would be best. However I do not agree with the decision the judge made for custody. I know that I have 10 days to put in an appeal. I have previously put in an appeal for child support which was fairly easy. I told the clerk that I wanted to appeal the decision and she gave me a paper to fill out that can be found online as well. Is there any such form that can be found online to appeal a custody case or do I actually have to write up a petition for an appeal. I know that every case is different. But with the child support appeal it was basically just filling in the blanks with my information and my reason for the appeal. Would this be the same for custody? And if so where can I find the form?

Just ask the clerk if the court has a form for the appeal of a final custody decision.
