Change of First Name

Am trying to change my first name and the documents ask if I have ever changed my name before. Not my first. But my last due to marriage. 1 problem tho...I forgot to change it on my ss card years ago. How do I write the paperwork out? Change or no change ever? With my maiden name or my maiden & married name hyphened? What will be on record? Do you think this will be to complicated for me to do alone?

You should fully answer the question and if you did use another name (which you did) it should be listed. While you may be able to do this on your own, you may feel more comfortable with an attorney guiding you through the process.
