Name Change

What is the process to change married name back to maiden name after a 1997 divorce.

You can pick up a packet of forms with

instructions entitled "Name Change For An Adult" from the clerk's office of certain circuit courts (civil division)

which will explain how it can be done without the aid of an attorney.

You may want to call first the particular court in your area to determine whether they have a supply of these packets on hand.
