changing a VA birth certificate
I am 19 and I was born and still live in VA. When I was born my parents were not married and my mother wanted to give me her maiden name. My Social Securtiy card has her maiden name as my last name, and I have always went by that name, but my birth certificate has my father's last name on it. my mom had sent it to be corrected but they always sent it back the same. Since I am now an adult, i don not have to have my father's signature to change it, but im not sure what I have to do to get it fixed. I am moving to another state in about 2 months so i need to get a new correct birth certificate ASAP. so what do I have to do?

Some clerk's offices of circuit courts
have a packet of forms with instructions entitled "Name Change for an Adult" which explains how to do the name change without engaging the services of an attorney.
The petition for the name change is filed with the circuit court which must approve it and then forms can be submitted to the Bureau of Vital Statistics in Richnond to record the official change.
