Child Name Changes

My step daughter is trying to have the name of her daughter changed from the non supporting father's last name. They were not married and he is not around. She cannot afford a lawyer and is trying to do it herself but cannot get anyone to provide advice on the procedures to do this. Can anyone provide a step by step way to do this? TIA

Yes, quite a number of circuit courts

carry an instructional packet with forms entitled "Name Change For a Minor" which explains how to do the name change without the need for an attorney.

These packets can be found in the clerk's office of those courts which have them. (Prince William County Circuit Court may have a supply on hand.)
