childs name change

My son is 2 years old, we have had no contact with the father since he was 3 months old and I recieve no support. He has his fathers last name, however he did not sign the birth certificate and no paternity test has been done. Can I change his last name to mine without contacting the father?

You can pick up a packet of forms with instructions from certain clerk's offices of circuit courts entitled "Name Change For a Minor".

This will explain how you go about filing for the name change without engaging the services of an attorney and whom must be given legal notice regarding the petition.
