corroborating witness in uncontested divorce

Greetings,

I have a question regarding a corroborating witness in an uncontested/no-fault divorce in VA. I'm representing myself and I'm requesting an Ore Tenus hearing. I know that I have to bring a witness to my hearing. I would like to know if my witness has to fill out a ''witness information form'', or sign any other forms? If yes, does it have to be done and filed along with the Ore Tenus request or it doesn't matter? And where can I get the forms if there are any?

Also, I'm aware that when representing myself, I am responsible for preparing my witness to answer the questions in the court. But I would like to ask, if I actually have to write down the questions and answers, and if my witness has to sign and notarize them before hearing, or there is no need to do that?

I would appreciate any information. Thank you very much in advance.

The answer is no to all of your questions. The witness you need to bring to the Ore Tenus hearing

is one who knows your personal situation vis a vis the spouse from whom you are seeking the divorce, and who can testify truthfully under oath to essentially confirm the various allegations laid out in your Complaint which constitute your grounds for divorce.

No special forms need to be filled out for this witness as far as I am aware.
