entrustment agreement

Hello

I and my husband live in Ghana West-Africa. My husband is an America citizen but i am not. I am here with my 21month old daughter and she needs to see doctors while she is here. I am pregnant and i have to get back home to have the baby but i need to leave my 21month old daughter with her grandparents. I need to sign off something so that they can take her to the hospital and get all access to her medical information, what do i do ? i just need something that can allow them access to all her medical information while i am gone and we will be back to pick her up in a couple of months. I leave in 3 weeks and when i called the social service here in Va they said what i need is call entrustment agreement? is that what i need?

Sounds right; contact the local bar association in your area for the names of a couple of family law attorneys who could draft up one of these kinds of agreements.
