I have full legal and physical custody of my 6yr son. I would like to change his last name to mine. How do I go about doing it? His father who he never sees would contest.

You should be able to pick up from the clerk's office of your local

circuit court (or download from the court's website) a packet of forms with instructions entitled "Name Change for a Minor Child" which will explain how

to do this name change on your own (pro se) and without an attorney.

The father of the child must be given proper notice of the hearing scheduled

in the matter and the opportunity to contest the name change.
