My husband and I are seperating and as part of our agreement he will keep the house and I will keep the business. Is there a form to outline that?

No, I don''t believe there is and this kind of an agreement should be formalized in a Property Settlement Agreement (PSA) by an attorney who is knowledgeable and experienced in such domestic relations matters.
