Legal Question in Family Law in Virginia

Judgement

I was recently sued by a dentist. He said I owed him $700. The guy is either a crook or someone made a mistake in his billing dept. We went to court the first time and I won. They appealed and we went to Judicial Circuit court. The dentist did not have any proof and he lied and now I have a judgement against me. I would like to appeal this but I am not sure which forms I need to fill out. I am in Chesterfield, Va. 23237.
