File petition for "Name Change for a Minor Child" in Virginia Beach Circuit Court. Contact the clerk's office (civil division) to determine whether they have the packet of forms with instructions which you can use for this filing without engaging the services of an attorney to do it.

If they do not have it available in the clerk's office, you may be able to download it from the court's website.
