Minor child custody in the event of custodial parent's death

I have sole legal and physical custody of my 6 year old daughter. Her father and I were never married, but his name is on the birth certificate. He signed a paternity acknowledgement form at her birth and she has his last name. We have been split up for 3 years now and he has not had consistent visitation or contact with her. I want to know if I can name my mother (who has helped me care for my daughter since her birth) as her guardian in the event that I die. Please help... Thank you!

Yes, you can name your mother as a legal guardian for your child.

You can obtain a brochure from your local court

instructing you on how to go about obtaining such

guardianship for your daughter.
