mother not listed on school and medical records

I am NCP-mom, recently I found out that my childrens stepmom is listed as ''mother'' on school and medical records. Is this legal? How can I get it changed? I have full access to records.

Take it up with school authorities as this issue would appear to involve administrative rather than legal issues (in my opinion).
