What exactly does it mean to terminate parental rights? Can a parent do this themselves? If so does a judge or the ex have a say in this?

Terminating parental rights is just what it says:

extinguishing all the rights that any parent has

to make decisions, whether major or minor, in the lives of his or her child(ren). Only a court of proper jurisdiction has the power to do this.
