Legal Question in Family Law in Virginia

Hi, I have a few questions about Virginia Addendum for Protected Identifying Information. At the bottom of the form, do we have to list our account numbers and such if we have nothing jointed anymore? Also, I moved to Florida, should I put my VA address or FL address? I was still in VA when I filled the complaint for divorce. Last, do I only check the box that says Decree or I should check something else like agreement of the parties?

Account numbers and related information are listed only for such documents as listed in the Privacy Addendum from which such information has been specifically referenced previously and deleted either by a party or a party's attorney.

Decree should be checked if the next step in your divorce is filing the

final decree of divorce with the court.

Use the same address as you used in filing your Complaint.
