taking maiden name back

divorced for years in florida and living in va.i am very ill and do not want married name when i pass on how to restore maiden name with very little money

You might be able to pick uo a packet of forms with instructions from the clerk's office of your local circuit court. It's entitled "Name Change for an Adult", and will tell you how to go about filing for the name change without the assistance of an attorney.

In addition to accessing information from the court you may wish to contact your local legal aid office to see if you qualify. If so, an attorney there would be able to assist you.
