What exactly does it mean when the custody order says that father is to have visitation as determined by the terms and conditions set by the mother from both a legal and practical standpoint?

Mom is the decider when Pop will visit his child(ren) and under what conditions such visits will take place, is the practical as well as legal import of this particular clause or provision of the custody order which you've referenced.

(And, if Pop isn't happy about this particular part of the custody order, he can go back to court and talk it over with the judge who approved and signed off on it.)
