Legal Question in Medical Leave in Virginia

FMLA question

I have been on FMLA for 8 weeks and was recently asked to fill out a packet of papers from the company I work for, I need to get my doctors signature and more information on my illness documented from him as well and my deadline to get it done is on Friday, the 13th. I can't be seen until the 17th, and if I don't get it filled out on time, can I lose my job? I am very concerned, Please help.
